Traditional Impression
Verify Your Impression
Check impressions for accuracy and quality. Sanitize and pack in a biohazard or sealed Ziploc bag. Wrap any models in bubble wrap before placing in the bag.
Complete the Rx Form
Fill out and sign the prescription form — include material choice and shade. Attach it to the OUTSIDE of the biohazard bag. Do not place the Rx inside the bag.
Package & Ship
You can include multiple cases in one box using a single shipping label. Schedule a pickup with UPS or FedEx, or drop off at your nearest location.
Send Photos (Optional)
Email any case photos to [email protected] and reference the doctor and patient names so we can match them to the case.
